    The secret one that people miss is 'batsman arriving late at the crease'.

    I heard them discussing it on Aunty a season or two ago when this question was posed. There is, apparently, some time frame in which the batsman must make his way to the crease. If I remember rightly, someone at test level has been given out this way, but only the one.
