The real problem is the idiot architects that put down lights in locations that require that sort of a solution on a regular basis.
Had a friend of mine whose architect brother designed a house for him with down lights in the apex of a double story cathedral ceiling. Looked great but required scaffolding to be erected just to change a bulb. In the end he only replaced the blown bulbs when he put the house up for sale.
